Ambulatory Clinical Pharmacist needed in Northwestern Ohio

We are looking for an experienced Ambulatory Care Clinical Pharmacist to serve our ambulatory patients as a part of our Health System's Physicians Group with a focus on Anticoagulation, Diabetes Management, and Wellness Visits. Precepting of pharmacy students and residents will also be available in our very supportive environment.

All candidates must have, at minimum, their Bachelors in Pharmacy and hold either a PGY1 Ambulatory Residency or 2 years equivalent recent or current clinic-based pharmacist experience. A PGY2 in Ambulatory Care is preferred.
