You'll collaborate with clients, their families, and our field staff to manage schedules, communicate needs, and update care plans. You’ll be a key part of a supportive, friendly office team.

*Key Responsibilities:*
* Handle client and caregiver inquiries
* Create and maintain schedules
* Resolve care and scheduling issues
* Collaborate with other team members to meet client needs
* Maintain documentation and ensure compliance
* Participate in paid rotating on-call coverage

*Requirements:*
* 1-3 years of relevant experience (Case Management, Home Care, Hospice, or Home Health preferred)
* Exceptional communication and problem-solving skills
* Strong attention to detail and ability to work under tight deadlines
* Homecare on-call experience and WellSky knowledge required
* Valid driver’s license and ability to pass background check

*Preferred Education:*
* High School diploma required; Associate or Bachelor’s degree preferred

Job Type: Full-time

Pay: $44,000.00 - $50,000.00 per year
