POSITION SUMMARY 
Social Worker, LCSW

POSITION DUTIES 
Seeking a compassionate and experienced travel social worker to support our transplant team. In this role, you will provide support, advocacy and guidance to transplant patients and their families throughout the transplant process. You will be responsible for completing comprehensive biopsychosocial assessments, participating in Selection Committee and collaborating with the transplant team to ensure coordinated and integrated care delivery. 
MINIMUM REQUIRED QUALIFICATIONS MA LCSW, BLS

PREFERRED QUALIFICATIONS Experience in transplant or organ donation preferred.
LENGTH OF ASSIGNMENT 13 Weeks

SHIFT / HOURS PER WEEK 8:00a - 4:30p (5x8)
