Required Qualifications
Licensed Practical Nurse, Long Term Acute Care
Experience: 3 years
Licenses: LPN(Compact Accepted)(Copy needed)
Certifications: BLS-AHA(Copy Needed)
SSN Required
DOB Required
References: 1 Reference in the last 12 months

SCL within the last year
This facility requires ALL work history information (Bed count, EMR, trauma level) to be in the facility name line before submitting
