Hourly pay range: $23 - $26
Maintains the delivery of health care services for individuals with developmental disabilities who are living in community based homes. Provides/participates in agency training.
ESSENTIAL FUNCTIONS/RESPONSIBILITIES
• Monitors the delivery of preventive and routine health care for individuals with developmental disabilities.
• Provides preventative, routine, and emergency healthcare for individuals with developmental disabilities.
• Adheres to and ensures facility compliance with all Physicians, NP, PA etc. orders.
• Ensures the scheduling and completion of all health-related exams, appointments, laboratory/diagnostic testing as ordered or required if assigned.
• Accompanies individuals on appointments, if assigned.
• Safely transports individuals if necessary.
• Completes medication, treatment, and supplement administration in the facility.
• Maintains compliance with all applicable state/federal regulations and agency policy and procedure.
• Maintains communication with Site RN, LPN Senior Residential Supervisor, Residential Director and other staff as necessary.
• Communicates all issues and carries out all directives of the overseeing RN.
• Assist in completion of daily logs, pertinent forms and documentation. Completes all nursing and program documentation promptly.
• Responsible for implementation of individual programs.
• Assist individuals as needed in all activities and areas of daily living.
• Completes all areas assigned through shift responsibility sheets to ensure health and safety of individuals.
• Performs housekeeping duties and laundry as assigned.
• Participates in pre-shift report meetings and house staff meetings as required.
• Attends all agency mandated in service training sessions in appropriate time frames.
• Attends all site specific trainings as mandated by the RN Clinical Coordinator and/or the Director of Nursing for the Agency.
• Required to fulfill pre-scheduled on-call commitment as determined by the Senior Residential Supervisor.
• Performs CPR and Strategies for Crisis Intervention and Prevention (SCIP-R).
• Complies with all agency policies and procedures.
• Other duties as assigned.
MINIMUM QUALIFICATIONS
• Current NYS license to practice as a Licensed Practical Nurse.
• Valid Driver’s License that meets agency policy.
• CPR certification.
• Ability to meet physical requirements of the position when performing the following: Standing, walking, sitting, bending, stooping, squatting, kneeling and climbing.
• Physical agility and ability to react to emergency situations, including maintaining Strategies for Crisis Intervention and Prevention Core Training certification as per regulatory guidelines.
