Nurse Case Manager - Ohio

Job Title: Nurse Case Manager

Job Location: Ohio

Type of Facility: Home Health Hospice

Length of Assignment: 13 weeks

Start Date: Immediately

Work Schedule: Monday-Friday, 8:30 AM - 4:30 PM

Roles and Responsibilities:

- Provide comprehensive and end of life care to patients with acute, terminal, and chronic diseases

- Deliver medical care in both home and inpatient settings

- Apply nursing process principles and implement therapeutic and preventive care

- Monitor and record vital signs

- Administer and document medications

- Educate patients on care plans

- Report significant changes in patient condition

- Manage a caseload of 5-7 patients per day

- Travel 25-40 miles daily

Requirements:

- BLS and CPR certifications

- Ohio RN License
