Nurse Case Manager - Ohio

Job Title: Nurse Case Manager

Job Location: Ohio

Type of Facility: Home Health Hospice

Length of Assignment: 13 weeks

Start Date: Immediately

Work Schedule: Monday-Friday, 8:30 AM - 4:30 PM

Roles and Responsibilities:

- Provide comprehensive care to patients with acute and chronic diseases in home and inpatient settings

- Apply nursing process principles and implement therapeutic and preventive care

- Monitor and record vital signs

- Administer and document medications

- Educate patients on care plans

- Report significant changes in patient condition

- Address patient and family learning needs

- Complete documentation accurately and timely

- Serve as a resource and provide expertise in end-of-life care

- Travel 25-40 miles per day (IRS mileage tracked in EMR)

Requirements:

- Valid Ohio RN license

- BLS and CPR certifications

- 1 year of hospice, home care, and/or community health experience preferred
