The Foundation for Sickle Cell Disease Research is the nation’s first comprehensive multispecialty center dedicated to sickle cell patients providing long-term, holistic services for those affected. The Foundation seeks the addition of a talented Family Nurse Practitioner for our Tallahassee, FL location.
SUMMARY
The Family Nurse Practitioner functions as a health care provider in an outpatient setting providing evidence-based care, including but not limited to diagnostic, therapeutic and preventative health care. The role is responsible for assessment, diagnosis, treatment, management, education, consultation, health promotion and care coordination. General hours are Monday through Friday, 8 a.m. to 4:30 p.m. Work is performed primarily in a clinical environment.
RESPONSIBILITIES
* Obtains complete medical history from patients and performs comprehensive and focused physical examinations appropriate for patient population.
* Conducts and documents comprehensive initial and ongoing physical examinations based on age and diagnosis, including psychological status.
* Organizes and presents historical and physical data, together with evaluations and recommendations, to the healthcare team in such way as to permit him/her to clearly identify a medical problem and determine appropriate diagnostic or therapeutic procedures.
* Synthesizes data and utilizes evidence to develop, implement, and evaluate the patient plan of care, including ordering and interpreting laboratory, radiology, and other diagnostic studies/results.
* Demonstrates proficiency in technical/procedural skills, as appropriate based on specialty and training.
* Executes diagnostic and therapeutic procedures as indicated.
* Provides follow-up and health maintenance care to patients based upon accepted standards of care and best practice.
* Instructs and counsels patients with respect to health promotion and/or health maintenance.
* Provides relevant management, referrals, counseling, and education to patients, families and caregivers related to current care and preventive health measures.
* Facilitates/participates in quality related initiatives through planned group processes, workshops, seminars, and staff meetings, to improve clinical and system practice performance.
* Maintains complete and accurate records of contacts with patients whether in person, virtually or by telephone.
* Collaborates in research projects and protocols; and routinely applies evidence based best practices to patient care. • Performs other duties as assigned.
LICENSURE/CERTIFICATION
• Current and unrestricted Florida licensure as a Nurse Practitioner in State of practice (APRN).
• Board Certification in Family or Adult/Geriatric Primary Care
• Current Basic Life Support (BLSA) and Pediatric Advanced Life Support (PALS) certified by the American Heart Association or Red Cross for Advanced Practice Providers.
REQUIREMENTS
* Master’s degree or higher in nursing, documentation of completion of accredited Advanced Practice Education Program. Graduate education and related Florida Board of Nursing authorization must be consistent with clinical practice area and patient population.
* At least 1 year post clinical experience with strong assessment skills.
* A high degree of critical thinking and clinical expertise in working with patients in an outpatient setting.
* Compliance with the COVID-19 vaccine mandate enforced by the Centers for Medicare and Medicaid (CMS) is a requirement for this position. Federal law requires individuals holding this position to be fully vaccinated or have an approved exemption for certain medical, disability, or religious reasons. Individuals who do not meet CMS vaccination requirements are not eligible and should not apply for this position but are encouraged to apply for other non-healthcare positions at UT Southwestern for which they qualify.

* Proficient use of EMR in the delivery of care
