The Nurse Practitioner will perform duties and responsibilities with the scope of practice for a Nurse Practitioner under the Colorado Nurse Practice Act. The Nurse Practitioner will assist with medical management in the outpatient areas. Will facilitate patient care management leading to efficient use of all health care provider clinic time and assure timeliness of clinic appointments. She/he practices independently according to established protocols and in collaboration with physician to deliver quality healthcare.
- Works in conjunction with the medical staff and nursing to facilitate patient management through the healthcare system, thus exercises independent judgment in the assessment, diagnosing and initiation of delegated medical processes or procedures.
- Performs and documents comprehensive health histories and physical assessments on assigned patients.
- Formulates diagnoses and identifies problems and health risk profiles of selected patients and documents and communicates this information to other members of the health care team.
- Performs orders and interprets diagnostic studies, such as x-rays and laboratory test, in consultation with staff as needed.
- Orders medication, treatments and therapies in accordance with the Colorado Nurse Practice Act.
- Requests consultation and initiates referrals to other healthcare providers.
- Provides information and support to patients and families regarding disease process and treatment plan, thus facilitating communication between patients/families and healthcare team.
- Educates patient and families regarding planned diagnostic procedures, medications, treatment and post-hospital care.
- Evaluates and consults with selected patients and families in outpatient setting at time of follow-up appointments regarding post hospitalized course of care.
- Facilitates communication between medical and nursing teams.
- HIPAA compliance.
Requirements
- Masters of Science in Nursing from an accredited program.
- Must be licensed in the state of Colorado to practice as a Nurse Practitioner.
- Must be licensed to have Prescriptive Authority.
- Three (3) years health care experience desired.
- Current BLS.
- Knowledge and experience utilizing a personal computer, EMR. Descriptive dictation for coding & billing purposes.
- Word processing skills required.
- Ability to work with and maintain confidentiality of patient, patient account, and personnel data.
- Ability to effectively communicate and work with physicians, patients, patient families, staff and management.
