General Job Description: The Critical Care Nurse Practitioner (CCNP) is a member of the Intensivist care team and is responsible for co-management of the critically ill complex patients in an Intensive Care Unit setting. The CCNP works collaboratively with the ICU Intensivist or designated alternative physician. The CCNP has specialized skills and knowledge related to the ICU setting, and performs physical exams, collects and analyzes data, formulates medical diagnoses, performs approved diagnostic and therapeutic procedures, orders and schedules laboratory studies and professional consultations, prescribes appropriate interventions and medications and provides direct patient care. The CCNP also assumes a leadership role in the Rapid Response Team and Code Blue Team. The CCNP contributes to excellence in patient care, research, teaching, consulting and providing leadership to the organization and nursing profession.

Duties and Responsibilities:

  • Provide direct clinical services for the critically ill adult patients according to established protocols and utilizes evidence-based clinical practice standards in collaboration with the attending Intensivist or designated substitute physician.
  • Provide emergency and urgent clinical services to any adult patient, employee or visitor via the Rapid Response team or Code Blue Team.
  • Obtain patient health histories and complete physical exams. Documents findings noting pertinent normal and abnormal findings. Complete both admission histories and physicals as well as Critical Care Consultations. Perform electronic documentation of daily progress notes, order entry and dictations according to standards.
  • Participate in daily radiology and multidisciplinary bedside rounding. Present patient cases during rounds in a scholarly and organized manner. Make revisions to plan of care as necessitated by daily rounding recommendations.
  • Order and interpret appropriate diagnostic and laboratory testes. Explain the necessity, preparation, nature and anticipated effects of procedures(s) to patients, patient's families and healthcare learners.
  • Perform approved therapeutic or diagnostic procedures based upon patient's clinical status. Document patient preparation and response to procedure(s).
  • Diagnose acute and chronic medical conditions.
  • Formulate and implement treatment plan for acute illness in collaboration with attending Intensivist or designated substitute physician. Document in the medical record and perform dictation. Utilize prescriptive privileges as allowed by the Pennsylvania State Board of Nursing. Document all discharge prescription in the patient's medical record.
  • Assess the patient for change in status and institute appropriate interventions. Initiate necessary emergency interventions to stabilize the patient.
  • Provide family-centered care by communicating with the family regarding the changing care needs of their family member. Assess family adaptation, coping skills and the need for crisis or other intervention.
  • Utilize current research findings and evidence-based decision making in all clinical practice. Incorporate clinical research findings in the development and implementation of standards of care.
  • Perform and participate in quality/performance improvement activities and clinical research.
  • Serve as a clinical expert and leadership consultant to nursing and members of the healthcare team.
  • Participate in and support accreditation, compliance and regulatory activities of the organization.
  • Precept nurse practitioner students, new nurse practitioners, and other advanced practice nurses and healthcare learners.
  • Participate in the development, review and evaluation of CCNP practice protocols.
  • Serve as a role model for advanced practice professional practice through active participation in professional organizations and continuing education. Enhance the body of knowledge for critical care nursing through the exchange of ideas and knowledge in professional organizations, oral presentations, posters, research activities and written publications.

Required Qualifications:

Education: Completion of an accredited Nurse Practitioner program. Bachelors

and Masters Degree in nursing required. Completion of ACNP post masters program strongly preferred if not specialty.

Experience: Five years as an RN with at least three years in Critical Care

Degrees/Licensure/

Certifications: Current licensure as a RN in the state of PA.

Current licensure as a CRNP in the state of PA

National Certification as an NP within 3 months of employment

Acute Care Certification strongly preferred

BCLS, ACLS Certification

AHA Stroke Scale Certification

FCCS certification prior to or within 1 year of employment

Must demonstrate and maintain all credentialing and licensing

requirements of Chester County Critical Care Associates, and the

Pennsylvania State Board of Nursing.
