Terrace Home Health is looking to find a highly qualified OT to join our innovative team to provide care and support for our patients. Your guidance, teaching and skills will provide not only a healing opportunity for your patients but their families and encourage each of them to reach their full potential.

For this role, we are looking for an Occupational Therapist who sees the value of home health care in alleviating pain, returning function and helping patients and families gain the skills and health needed to reach their full potential. A compassionate nature and excellent interpersonal skills will make you a good candidate for this position. Experience in Home Health is preferred (but not required).

Responsibilities:

* Collaborate with physicians and other professionals to plan care
* Make frequent visits to the patient to evaluate their condition
* Provide support and comfort according to individual patient’s needs
* Work with care team, patients, caregivers to plan and execute excellent patient care
* Monitor, manage, alleviate and improve symptoms of disease
* Notify a doctor for a patient’s condition when deemed necessary
* Maintain accurate records
* Teach and guide staff team members in assisting with patient care and treatment

Requirements and skills \*OT Management of patients and family

* Communication with the Interdisciplinary Team
* Utilizing EMR (electronic medical record)
* Patient and Family Education on disease process and treatment
* Prefer one year of experience in home health
* Must have valid OT license in good standing with the State of Missouri and/or Kansas
* Driver’s License
* Able to Pass Various Background Checks
