What You’ll Do:
Work in collaboration with the Palliative Care Medical Director and other palliative team members to deliver palliative care by providing diagnosis, treatment and follow-up for patients referred to palliative care.
Care for patients in their home within a 60 mile radius of our Tigard, OR. office
Provide regular visits to reassess the patient’s needs, concerns, and update the plan of care
Initiate communication with the Primary Care physician, other agencies involved in patient care
Prescribe medications including controlled substances to the extent delegated and licensed, in accordance with state law.
Order and interpret laboratory and other diagnostic tests within the scope of professional practice.
Order treatments and durable medical equipment as indicated
Qualifications:
Licensed as a Nurse Practitioner in the state of practice
Minimum of Masters of Science in Nursing from an accredited school of nursing, or an equivalent as allowed under state law
Training/experience in Palliative Care, Gerontology, Geriatric Medicine and/or Chronic Care preferred
Prior Home Health and/or Hospice experience as an RN or NP preferred
Practice experience with Part B billing preferred
Have current DEA license
ACHPN – Certification in Hospice and Palliative Care Preferred
Minimum of two years of varied work experience as a professional nurse
Must possess current CPR Certification
Demonstrated skill in nursing practices and clinical judgments
Ability to make home visits and provide nursing care in the home environment
Ability to cope with family emotional stress and tolerance of individual lifestyles
Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
Valid driver’s license and proof of insurance is required
