1) How long is the contract assignment? 13 weeks 2) Schedule of assignment hours? M – F, 8am – 5pm And every other weekend 3) Will this candidate be visiting patients in care facilities or only in patient’s homes? Patients homes and assisted living facilities 4) Will you want to hire the contractor at the end of the contract or is this truly a contract assignment? Contract assignment 5 (CL will direct on pricing) 6) What region are you in? Home health or Hospice? Southeast South – Home Health 7) What’s creating the request? - work load - Full time RN is going PRN 8) To which location will this person report for assignment, full address, Including the OU (Location Code) Campbellsville 2152 – 175 West Beartrack rd Campbellsville Ky 42718 I also cover Greensburg 2151 -102 South Main Street Greensburg Ky 42743 which is only 15 min from the Campbellsville location - please add this care center also just in case they have to see patients here. Please confirm above location is correct and list any other territories that are to be covered by the candidates: 9) How many miles per week will the consultant travel? 30/40 a day? Weekly? 10) What are the weekly units of productivity expectations? 25/30? 30 11) What EMR systems will the consultant be using: HomeCare HomeBase, Oasis, Outlook 12) Will there be any on call? If so, how much? Every other weekend and 2 days during the week 13) Do you want the candidate to have home health experience? yes
