As an RN Case Manager, you will play a critical role in making our patients' final days, weeks, and months as comfortable as possible. *Responsibilities:*
* Conduct initial client assessments and continuously refine care plans.
* Provide hands-on nursing care and patient education.
* Maintain open lines of communication with patients, families, and the healthcare team.
* Manage patient care coordination for your caseload.
* Participate in interdisciplinary team meetings to ensure comprehensive care planning.

*Qualifications:*
* Licensed as a Registered Nurse / RN in the state you work in.
* Proficiency with Electronic Medical Records (EMR) software.
* Excellent planning, organizing, and communication skills.
* Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
* Valid Driver's License and reliable vehicle with current insurance.

Pay: $75,000.00 - $90,000.00 per year
