POSITION SUMMARY - RN Case Manager

POSITION DUTIES - Case Manager responsible for arranging all post-acute needs for patients

MINIMUM REQUIREMENTS - 

• Acute Care case management experience/discharge planning experience
• Interqual/Milliman experience
• Familiarity with California market
•  Previous HCA case management experience
• At Least 2 years experience

LENGTH OF ASSIGNMENT - 13 Weeks
SHIFT / HOURS PER WEEK - Day Shift: 8am - 4:30 PM
SYSTEMS - Meditech and Midas

START DATE - 12/16/2024
