POSITION SUMMARY - RN Case Manager

POSITION DUTIES -

• Skilled observation & assessment
• Wound care
• Injections & IV therapy
• Nursing Case Management

MINIMUM REQUIRED QUALIFICATIONS - Minimum of 2 years current Home Health experience in Case Management and Hospice Experience 

LENGTH OF ASSIGNMENT - 13 Weeks

SHIFT / HOURS PER WEEK - 5 days/week Mon - Fri 40hr guarantee

SYSTEMS - Netsmart/Allscripts EMR experience strongly preferred

START DATE - 12/16/2024
