POSITION SUMMARY - RN Case Manager

POSITION DUTIES - Discharge planning and care coordination

MINIMUM REQUIRED QUALIFICATIONS - 5+ years experience, RN License, BLS, Acute Care Experience

LENGTH OF ASSIGNMENT - 15 Weeks

SHIFT / HOURS PER WEEK - Weekends, 10 hour shifts, Friday-Monday

START DATE - 1/6/2025
