POSITION DUTIES - Care Coordination / Continuum of Care / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS - RN License / BLS / Acute Care Experience

LENGTH OF ASSIGNMENT - 13 weeks

SHIFT / HOURS PER WEEK - 8:00AM - 5:00PM / 40

SYSTEMS - Meditech
