*Service Areas: * Two openings! One in greater Londonderry communities and one in city of Nashua, NH!

*Responsibilities*
* Completes an initial home health assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
* Utilizes the nursing process in planning, providing and evaluating skilled needs.
* Completes a comprehensive patient assessment that is reflected in accurate OASIS scoring.
* Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and family on the planning and revising process.
* Case management of your patients! You are key to helping them heal, getting back to feeling strong and healthy!

*Qualifications*
* Graduate of an accredited school of nursing.
* Associate degree in nursing.
* Registered as RN with New Hampshire’s Board of Nursing.
* A minimum of two years’ recent acute care experience.
* Ability to exercise initiative and independent judgment.

*Preferred Qualifications*
* Bachelor’s degree in nursing.
* Community Health/Education experience.
* Certifications: IV, Chemotherapy, Ostomy, as examples.
* Familiarity with OASIS and what drives reimbursement.
* EMR software proficiency.

*Required*
* Registered as a RN with New Hampshire’s Board of Nursing.
* CPR Certification.
* Satisfactory criminal record.
* Satisfactory motor vehicle record.
