*Job Title: RN - Clinic (HARP)*
*Location: Boston, MA *
*Duration: 4 months (possible extension)*
*Shift Details: 40hrs, Monday –Friday 8-5PM*

* Department Name: Hospital Admission Reduction Program (HARP)
* Mix of home visits and on campus work
* It is about 2-3 home visits per day and depending on patient’s location, with weekly mileage averaging between 35 to 45miles.
* Holidays Requirement: No major holiday commitment
* Weekend Requirement: No weekend commitment
* On Call Requirement: RN may serve as coverage for other Dyad RN as needed/when needed
* Number of Beds: Not Applicable
* Average Patient Ratio: 35-40 Patient Panel Size
*Experience Qualifications:*
* Years of Experience: Minimum of 3 Years of Ambulatory experience, working with Adult Nursing-65 years and older patient population. Preferred ambulatory/community based nursing experience.
* Required Certifications: MA RN LICENSE, BLS, ACLS, must be licensed to drive in MA, have a reliable vehicle for home and community visits.
* Preferred Experience: Inpatient experience, Geriatrics population, Care Management, Master’s prepared RN is preferred (but not required).
*Additional Comments/Details:*
* RN primary work is community based doing patient assessments in patient’s homes, but extends as needed into the inpatient and ambulatory settings in order to support transitions of care for patients returning to community and facility settings from Client.
* RN must possess clinical expertise in common high-risk medical conditions (e.g. CHF, diabetes, COPD, etc.), familiarity with home health and community-based resources, as well as excellent interpersonal skills, patient engagement skills and the ability to work independently and collaboratively.
* RN must be licensed to drive in MA, have a reliable vehicle for home and community visits.
* Must be able to collaborate with community partners, such as VNA agencies, caregiver programs (PCA, ADH, AFC), DME providers, social service agencies, and skilled nursing facilities.
* Must be able to identify, assess, triage, and rapidly respond to clinical changes that could lead to the need for emergency services if not intervened upon.
* RN works closely with Community Health Workers, pharmacy liaison, and/or Social workers.
* RN must be able to meet regularly with leaders at the local clinical site (Primary Care, ED, inpatient), and care management supervisor, to triage program issues appropriately.
* RN must be able to participate in local site operations, including team meetings, curbsides with care team members, etc.
* RN must be able to facilitate interdisciplinary consultation on patient’s behalf through participation in rounds, team meetings and clinical reviews
* RN must comply with established metrics for performance and adheres to documentation and workflow standards.
