Job Title: Social Worker

Job Location: California

Length of Assignment: 26 weeks

Start Date: Early March

Work Schedule: 9a - 5p shifts for 4 days per week

Roles and Responsibilities:

• Assess patients' psychosocial needs and develop appropriate care plans

• Provide counseling and support to patients and families

• Assist patients with discharge planning and coordination of community resources

• Maintain accurate and timely documentation of patient interactions and interventions

• Handle substance abuse, 12 step programs, and group therapy

• Participate in case conferences and team meetings

Requirements:

• LCSW certifications

• Current California state licensure

• Experience with substance abuse, 12 step programs, and group therapy
